For tax and catalog purposes, a separate form must be completed online for each donation.
Please include a clear description and restrictions such as expiration dates and quantity limits. If an item was donated by a business, list the TLS contact as the solicitor and the business as the donor.
*Note: we are unable to accept used items, percent-off coupons toward services or multi-level-marketing parties.
